Create a medical invoice by following these steps:
- Download your free medical invoice template
- Add your contact information: company name, your name, address, email address and phone number
- Insert your company logo at the top
- Fill in your patient’s or insurance company’s contact information
- Add an invoice number, invoice date and payment due date
- List the services provided and details of each. Be as specific as possible.
- Add a diagnostic code for each service. Some providers limit how many codes you can bill per patient visit
- Add prices for each service. Find out the billable amount for each service if you’re working with Medicare or an insurance company. Bill the patient for the remainder, if necessary, or write it off
- Make sure you comply with provider requirements. Figure out if you need to bill a patient a deductible, coinsurance or copayment
- Add sales tax, if required by your state
- Add anything that affects the total such as discounts, balance owing, credits etc.
- Calculate the total
- Insert payment terms such as late fees and accepted payment methods
- Send the completed billing invoice to your client online via email. Or print it off and send via regular mail
